Accident Summary:
While hovering out of the POL area, the pilot ran out of left pedal. The aircraft made 3 turns to the right before hitting the ground. The tail rotor blade hit the tail light resulting in incident damage

War Story:
Hovering over water from the POL point to the revetments made the standard "slide over when over gross" impractical. We usually hopped over, so to speak. In this case the three 360s necessary to keep the aircraft out of the water raised my pucker factor considerably and made it difficult to keep my slide on area in sight. I thought of it as a successful spinning slide on without co-pilot (left him on pad to lower weight, I think) and a possible new training maneuver to be demonstrated to newbys. The incident report did not note the fact that upon stopping the aircraft, the skids had spread so that I had immediate direct exit from the cockpit almost to the ground - also an improvement as I did not like that big jump down. I have some nice pictures (somewhere) of my newly "lowered" Cobra. Note also the low cost of my incident $2,383--You just can't get that kind of value these days! CW4 John M Knox, Retired

Accident Summary:
CW2 RYAN AND CW2 SAYLES LANDED THEIR AIRCRAFT AT BEARCAT FOR FUEL ALONG WITH AN LOH. THEY REFUELED THEIR AIRCRAFT TO 1340 &OUNDS OF FUEL. THEY REMAINED AT POL WHILE THE LOH REPOSITIONED TO ANOTHER PART OF THE BEARCAT COMPOUND.
APPROXIMATE 10 MINUTES LATER (1200 POUNDS OF FUEL SHOWING), CW2 RYAN CALLED BEARCAT TOWER FOR TAKE-OFF INSTRUCTIONS. THEY WERE CLEARED ONTO THE RUNWAY AND REPORTED THRIR HOVER CHECK COMPLETE. EVERYTHING APPEARED NORMAL. THE TOWER OPERATOR CLEARED THE AIRCRAFT FOR TAKE-OFF. UPON REACHING AN APPROXIMATE ALTITUDE OF 70 TO 100 FEET AND AN INDICATED AIRSPEED OF 40 TO 45 KNOTS. THE AIRCRAFT HAD AN ENGINE FAILURE. AT THIS POINT THE COBRA WAS OVER THE ROAD THAT CROSSES BEARCAT ACTIVE. ALTHOUGH CW2 RYAN HEARD NOTHING BUT THE ENGINE DYING, CW2 SAYLES AND A WITNESS ON THE GROUND (PFC JONES) STATED THAT THEY HEARD A LOUD BANG AN THE ENGINE. WITNESSES ALSO SAW A LONG FLAME SHOOT FROM THE EXHAUST. CW2 RYAN SAID HE BEGAN LOWERING THE COLLECTIVE AND ROLLING OFF THE THROTTLE, WHILE NOSING THE AIRCRAFT OVER TO TRY AND GAIN SOME AIRSPEED. THE AIRCRAFT DESCENDED NOSE LOW, MAINTAINING RUNWAY HEADING. AT ABOUT FIFTEEN (15) FEET ABOVE THE GROUND, CW2 RYAN EXECUTED A SLIGHT FLAIR AND THEN LEVELED THE AIRCRAFT. THE AIRCRAFT HIT THE RUNWAY, SPREAD THE SKIDS, AND CONTINUED TO SLIDE ON RUNWAY HEADING. JUST BEFORE STOPPING, THE AIRCRAFT TURNED TO THE LEFT. IT COULD NOT BE DETERMINED EXACTLY WHEN THE TAIL BOOM WAS SEVERED BUT IT APPEARS THAT THIS OCCURRED JUST PRIOR TO THE AIRCRAFT TURNING. CW2 RYAN STATED THAT HE STILL HAD DIRECTIONAL CONTROL BEFORE THIS. BOTH PILOTS EXITED THE AIRCRAFT AND THE CRASH RESCUE TEAM ARRIVED ON THE SCENE.
